Major Medical Insurance is available for horses from 1 month to 15 years by endorsement to your Full Mortality Insurance. LIMIT: $7,500.00 in the aggregate per insured animal per period of insurance. COVERED CHARGES: Medical Treatment: Reasonable and customary charges for the area by a licensed veterinarian necessitated by accident, injury or illness, including Surgical Expense. DEDUCTIBLE: $250 each separate claim during the Period of Insurance. EXCLUSIONS: Accident, injury or illness contracted prior to the effective date of this endorsement and any recurrence thereof. Any congenital birth defect such as umbilical hernia, contracted tendons, etc., whether evident or not at the inception of the policy and whether or not deemed developmental or acquired; Any treatment normally associated with the maintenance of a healthy animal; or any elective surgery including but not limited to castration, caslick's and cosmetic surgery. Any procedures or performance enhancing treatment such as chiropractic, massage, acupuncture, corrective shoeing, and intramuscular or intra-articular infections of synovial fluid stimulators or replacers (i.e.-Adequan, Hylauronic Acid, Legend, Corticosteroids, etc.,)whether or not used in conjunction with other treatments. Any treatment or diagnostic procedures for non-surgical lameness incurred after 6 months from the onset of the condition or coverage expiration, whichever comes first; Malicious or willful injury or poisoning or gross negligence whether or not caused by the insured; Expenses incurred for other than necessary medical treatment, such as veterinary travel or animal transportation fees. Any animal used for racing or intended for racing; Any animal under 30 days or over 15 years of age; PAYMENT OF CLAIMS: For the purpose of this endorsement only, you must file with us or our agency at your expense, proof of claim which is: A report signed by the treating veterinarian describing the medical treatment performed and describing the animal's condition and Copies of all service bills for which the claim is made, and Filed within 60 days after the medical treatment. All other terms and conditions of the policy remain unchanged. The annual premium is $250.00 per animal insured. This is a brief summary of benefits.
